3. Representations and Acknowledgments
By executing this eligibility attestation, I certify, acknowledge, and agree to the following:
A. Eligibility
Each individual counted toward the minimum 20% eligible ownership requirement satisfies the applicable military service or surviving-spouse criteria and actively participates in managing the business.
B. Verification
The business agrees to complete the Firm's eligibility verification process. One accepted verification document is required for each eligible individual counted toward the ownership requirement.
Accepted documents include, as applicable:
- DD Form 214;
- NGB Form 22;
- VA Veteran Identification Card;
- VA service-verification or benefits letter;
- State-issued identification bearing an official veteran designation;
- Statement of Service;
- Redacted Leave and Earnings Statement;
- Current military orders;
- Redacted DFAS Retiree Account Statement;
- Military retirement orders; or
- Notice of Eligibility for Retired Pay (20-year letter).
A screenshot or PDF of a digital VA Veteran Identification Card is accepted. Verification documents must be submitted separately by email to client@carbyneaccounting.com.
C. Veteran Status
For former service members, I acknowledge that the Firm requires an Honorable or General (Under Honorable Conditions) discharge, except that the Firm accepts state-issued identification bearing an official veteran designation as presented.
D. Surviving-Spouse Eligibility
Where surviving-spouse eligibility is claimed, I certify that the deceased owner satisfied the applicable military service requirements and that the eligible surviving spouse meets the business ownership and active-management requirements. I understand that eligible ownership may be recognized provisionally during estate administration when the surviving spouse is the reasonably expected recipient of the applicable ownership interest. Remarriage does not affect eligibility. During estate administration, this attestation may be executed by the estate's personal representative or another individual legally authorized to act on behalf of the business.
E. Notification Requirements
The business agrees to notify the Firm in writing at client@carbyneaccounting.com within thirty (30) calendar days following a material change in ownership, active management, or military service status affecting eligibility. Changes resulting directly from the death of an eligible owner are subject to a ninety (90) calendar day initial notification period. Material changes resulting from the final distribution of an estate must be reported within thirty (30) calendar days.
F. Reverification
At each renewal of the client's services, the business must re-execute this eligibility attestation. Currently serving individuals, including active-duty, National Guard, and Reserve personnel, must also submit a current verification document. I understand that failure to complete reverification by the renewal date may result in suspension of the discount. Following successful reverification and written approval, the discount resumes with the next applicable invoice issued after approval. Invoices issued during the suspension will not be retroactively adjusted solely because reverification is subsequently completed.
G. Discount Application
I understand that the ten percent (10%) discount applies only to the Firm's professional service fees. The discount begins with the first applicable invoice issued following written approval, is not retroactive, cannot be combined with other discounts, and excludes third-party and non-service charges. I acknowledge that the Firm is not required to delay invoicing while verification is pending.
H. Timely Notification
I acknowledge that, when timely notice of a change affecting eligibility is provided, the discount terminates beginning with the first invoice issued after the Firm receives notice, unless continuing eligibility is established. Discounts applied before receipt of timely notice will not be recovered solely because eligibility ceased during the applicable notification period.
I. Untimely Notification
I expressly acknowledge and agree that, if the business fails to provide the required notice within the applicable notification period, the Firm may recalculate affected invoices issued after the date eligibility ceased and seek recovery of the resulting discount adjustments, to the extent permitted by applicable law and the terms governing the client's services. I understand that late notification alone does not automatically constitute knowing or reckless material misrepresentation.
J. Material Misrepresentation and Recovery
I expressly acknowledge and agree that, if the business or its authorized representative knowingly or recklessly provides materially false or misleading information concerning eligibility, the Firm may revoke the discount, recalculate affected invoices, and recover discounts improperly received, to the extent permitted by applicable law and the terms governing the client's services. I understand that ordinary eligibility changes and good-faith inaccuracies are not independently treated as knowing or reckless material misrepresentation.
K. Privacy, Redaction, and Documentation
I consent, on behalf of each eligible individual and with that individual's authorization, to the Firm's evaluation of the minimum documentation reasonably necessary to determine eligibility. I understand that supporting verification documents must be submitted by email to client@carbyneaccounting.com.
Before submission, the business is responsible for redacting Social Security numbers, driver's license and identification numbers, dates of birth, home addresses, financial amounts, military assignments or locations, medical information, and other sensitive information not required to establish eligibility.
The Firm does not accept Department of Defense-issued military identification cards, including active-duty, retiree, or dependent identification cards. I agree not to email photographs, scans, or copies of such cards. I understand that the Firm may delete submissions that contain prohibited identification cards or inadequately redacted information and request corrected documentation.
Verification emails and supporting documents will be removed from the Firm's active systems following the eligibility determination, subject to applicable legal, regulatory, or preservation requirements. Residual copies may remain in system backups, email archives, or other provider-managed retention systems until those copies expire under the applicable service provider's retention schedule. The Firm will retain the administrative verification record and executed eligibility attestation as necessary to administer the program and enforce this attestation and the terms governing the client's services.
L. Incorporation into Client Terms
This eligibility attestation is incorporated into and governed by the terms governing the client's services with Carbyne Accounting LLC and by the Armed Forces Service Discount Policy. By executing this attestation, the client acknowledges and agrees to the eligibility requirements, verification procedures, notification obligations, and provisions governing material misrepresentation and recovery of improperly received discounts.
4. Electronic Signature and Consent
By electronically signing this attestation, I consent to the use of electronic records and signatures for this attestation. I acknowledge that my electronic signature constitutes my agreement to the representations, acknowledgments, and obligations contained in this attestation, including the provisions governing notification, material misrepresentation, and recovery of improperly received discounts.
I understand that the Firm will retain the electronically signed attestation and its submission record, including signer identity, execution timestamp, IP address, browser and device information, and associated information collected during submission, in accordance with the Firm's applicable recordkeeping procedures.
I certify that the representations made in this attestation are true, accurate, and complete to the best of my knowledge, following reasonable inquiry. I further certify that I am authorized to execute this attestation on behalf of the business and that any required authorization has been obtained from each eligible individual whose information is provided.
